The rapid entrance of LESS into the clinical realm is likely due to its similarity to standard laparoscopic techniques and the currently available instrumentation. In contradistinction the limited clinical application of NTES has been almost exclusively confined to general surgery. This is in part because unlike general surgery urology has no straightforward routine intra-abdominal procedure to which NTES might be readily applied. The available instrumentation for LESS is slightly better than for NTES but it is far from ideal. Designing purpose-built instrumentation and operating platforms is critical if LESS or NTES is to be promulgated in the surgical community. If NTES is destined for clinical application in urologic surgery it may very well be via a transition through LESS. ne can certainly imagine a small flexible robotic platform that could be inserted through a small abdominal incision to perform LESS and eventually through a natural orifice to perform NTES.
